މި ފޯމަކީ އއ. ބޮޑުފޮޅުދޫގެ ރަށްވެއްސަކަށް (ރަށުގެ ރަސްމީ ދަފްތަރުގައި ރަޖިސްޓްރީވެފައިވާ މީހަކަށް) ވުމަށް އެދި ކައުންސިލް އިދާރާއަށް ހުށަހަޅަންޖެހޭ ރަސްމީ ފޯމެވެ. ދިވެހިރާއްޖޭގެ ލާމަރުކަޒީ ގާނޫނާއި ރަށްވެހިކަމުގެ ގަވާއިދުގެ ދަށުން، އެހެން ރަށެއްގެ ރައްޔިތަކު ބޮޑުފޮޅުދޫގެ ރަށްވެއްސަކަށް ބަދަލުވުމަށް ނުވަތަ ރަށުގެ ދަފްތަރުގައި ރަޖިސްޓްރީވުމަށް އެދޭނަމަ މި ފޯމު ފުރިހަމަކުރަންޖެހޭނެއެވެ.
