މި ފޯމަކީ އއ. ބޮޑުފޮޅުދޫއިން ދޫކުރެވިފައިވާ ގޯތިތަކާއި، އެ ގޯތިތަކުގައި ކުރެވިފައިވާ އިމާރާތްތައް ގާނޫނީގޮތުން ރަޖިސްޓްރީކޮށް، ގޯތީގެ ރަޖިސްޓްރީ (އޮރިޖިނަލް ސެޓްފިކެޓް) ހޯދުމަށް ނުވަތަ ރަޖިސްޓްރީއަށް ބަދަލުގެނައުމަށް އެދި ކައުންސިލް އިދާރާއަށް ހުށަހަޅާ ފޯމެވެ. ދިވެހިރާއްޖޭގެ ބިމާބެހޭ ގާނޫނުގެ ދަށުން ބިމާއި އިމާރާތުގެ މިލްކުވެރިކަން ކަށަވަރުކުރުމަށްޓަކައި މި ރަޖިސްޓްރީ ހެދުމަކީ ލާޒިމްކަމެކެވެ
