Birth Certificate Application
This form is used to apply for a new birth certificate for a child born in AA. Bodufolhudhoo, submitted to the Council Office. Under the Maldives' Child Rights Protection Act and the Registration Regulation, it is mandatory for parents/guardians to obtain a birth certificate for their child within the prescribed period after birth.
